Digoxin for Heart Failure and Atrial Fibrillation; Digifab for Digoxin Toxicity

Overview

Digoxin is a cardiac glycoside used primarily to treat certain heart conditions by improving heart muscle contractions and controlling heart rate. It is available as oral tablets for long-term management of heart failure and atrial fibrillation, and as a specialized injection called Digifab for treating life-threatening digoxin toxicity or overdose.

What is it used for?

  • Digoxin tablets: Used to treat mild to moderate heart failure in adults, increase heart muscle contractility in pediatric heart failure patients, and control resting ventricular rate in adults with chronic atrial fibrillation.
  • Digifab injection (digoxin immune fab): Indicated for patients with life-threatening or potentially life-threatening digoxin overdose or toxicity, including acute ingestion of fatal digoxin doses, severe arrhythmias, high serum digoxin or potassium levels, and progressive cardiac symptoms not responsive to other treatments.

Active ingredients

Digoxin

How it works

Digoxin works by increasing the force of heart muscle contractions, which helps improve the pumping efficiency of the heart in heart failure. It also slows electrical conduction through the atrioventricular node, helping to control the heart rate in atrial fibrillation.

Digifab is an antibody fragment derived from sheep that binds specifically to digoxin molecules, neutralizing their toxic effects and allowing safe excretion from the body.

Dosage and administration

Digifab (injection): Dosage depends on the amount of digoxin ingested or the serum digoxin concentration and patient weight. For unknown acute overdose, 20 vials are given initially (split dosing may be used). For chronic toxicity without serum levels, adults and children ≥ 20 kg receive 6 vials; infants and children

Digoxin tablets (oral): Dose tailored by patient's age, weight, renal function, and concomitant medications. Loading doses can be given if rapid effect is needed, starting with half the loading dose followed by quarter doses every 6-8 hours twice. Maintenance dose in adults and children over 10 years starts at 3.4 to 5.1 mcg/kg once daily. Pediatric dosages vary with age and renal function. Serum levels should be monitored to avoid toxicity. Dose adjustments are necessary for renal impairment.

Contraindications

  • Digifab: No known contraindications.
  • Digoxin tablets: Contraindicated in patients with ventricular fibrillation and those with known hypersensitivity to digoxin or other digitalis preparations.

Warnings and precautions

  • Digifab: Monitor for rapid decreases in serum potassium after administration; risk of hypokalemia. Watch for anaphylaxis, especially in patients allergic to papain, papaya, pineapple enzymes, or sheep protein. Hypersensitivity reactions require immediate treatment and discontinuation. Serum digoxin assays become unreliable after Digifab administration. Patients with renal failure require close monitoring due to possible prolonged elimination and recurrent toxicity.
  • Digoxin tablets: Risk of ventricular fibrillation in patients with accessory AV pathways such as Wolff-Parkinson-White syndrome, especially if atrial fibrillation develops. May cause severe bradycardia or advanced heart block; consider pacemaker if indicated. Digoxin toxicity manifests as nausea, visual disturbances, and cardiac arrhythmias, and is more likely with renal impairment, electrolyte imbalances (hypokalemia, hypercalcemia), low body weight, or advanced age. Avoid use in acute myocardial infarction and myocarditis due to risks of ischemia and pro-inflammatory effects. Use caution prior to electrical cardioversion, and avoid abrupt discontinuation. Carefully assess risk-benefit in each patient.

Side effects

  • Common signs of digoxin toxicity include nausea, vomiting, anorexia, visual disturbances (such as blurred or yellow vision), and cardiac arrhythmias ranging from bradycardia to life-threatening tachyarrhythmias.
  • Hypersensitivity reactions to Digifab can include rash, urticaria, bronchospasm, hypotension, and anaphylaxis.

Drug interactions

  • Digoxin blood levels and effects can be influenced by various factors including renal function, concomitant medications, and electrolyte disturbances.
  • Digifab may interfere with serum digoxin immunoassays, leading to misleading serum digoxin concentrations after administration.

Overdose information

Digoxin toxicity includes symptoms such as nausea, vomiting, visual disturbances, and cardiac arrhythmias which require medical assessment. Digifab is used as an antidote for life-threatening digoxin overdose.
